Student medication
IMPORTANT! Any medication (including over-the-counter medication) brought onto school grounds must be approved by Health Office staff (email Health Technician (klloyd@srcs.k12.ca.us) or call 707-890-3820 ext. 54134). Please read the following guidelines (California Code of Regulations, Title 5, Article 4.1 and California Code, Education Code - EDC § 49423).
Each year, if your student takes medication at school, we need a Medication Authorization Form filled out completely for each medication. Please bring all medications and any other supplies your student needs to the Maria Carrillo High School Health Office in addition to the Medication Authorization Form(s) and Emergency Action Plan(s).
Student emergency care plan (ecp)
Each school year, we need an updated action plan, in case of emergency, for our students with medical needs. Please have the form completed by your medical provider and signed by you and the provider.

Immunization AND Medical exemptions
Beginning July 1, 2016, new legislation no longer permits immunization exemptions based on personal beliefs for children in childcare, public, and private schools. Students must have proof of vaccinations or a medical exemption. No personal belief exemptions will be accepted after December 31, 2015. The law allows personal belief exemptions submitted before January 1, 2016 to remain valid until a) the pupil reaches all kindergarten programs (Pre-K, TK, and KA), or b) the pupil begins 7th grade.
CAIR-ME Medical Exemption Process
Starting January 1, 2021, all new medical exemptions for school and child care entry must be issued through CAIR-ME.
- Parent/Guardian creates an account in CAIR-ME and applies for a medical exemption (ME).
- Parent/Guardian will receive an exemption application number to give to their child’s physician.
- The physician registers for an account in CAIR-ME and logs in to issue the ME.
- Once the ME is issued, the physician prints the form and provides a copy to the parent/guardian to give to their child’s school or child care facility.
