MCHS Athletic Packet 

 

 

     Any MCHS Athlete trying out for a sport must complete an athletic packet.  Each athlete only needs to turn in one packet for the entire year.  Here you can print a PDF format of the Athletic Packet.  Print out the packet and follow the instruction on the first page (also found below).

  • Get a physical dated June 1 or later of the current school year.  Bring verification of this physical, dated AFTER June 1, 2010 (This physical is good until July 1 of the following year.), including a doctor’s signature on the form provided, or on your doctor’s or Osteopath’s stationary, or on a Kaiser form.  If the doctor does not use the District Physical Form, all information must be attached to the District form and the rest of the Physical Form must be completed and signed.  If you have played or tried out for a sport this school year, you have a physical on record and do not need to turn in a second copy.
  •  Complete ALL FORMS and provide all information, including both student and parent/guardian signatures, within the District Athletic Clearance Packet.  Failure to do so will result in students’s inability to try-out.
  •  All students must provide evidence of health insurance including policy number prior to try-outs.  Students may purchase Athletic Insurance through the school (get information from the business office).
  •  The transportation fee of $50 (checks made out to Maria Carrillo High School) must be paid when you hand in your packet.  Please write a seperate check for each athlete in the family.  Transportation checks will be returned to those students who are cut from a team or who quit during the first four weeks of the sport season.  A hardship waiver is available for those student-athletes with extenuating circumstances.  See your business office for information on waivers.  If you plan to donate to PAC please write a seperate check.

  • PROCEDURE FOR CLEARANCE: 
  • 1.     Complete and sign all items in the packet. 
  • 2.     Return the packet to the appropriate athletic director listed below.
  • 3.     The athletic director will process the packet and fill out a white clearance form. 
  • 4.     It is your responsibility to pick up the white clearance from the athletic director . 
  • 5.     The coach will collect the white clearance form on the first day of practice. 
  • 6.     NO WHITE CLEARANCE FORM       NO PRACTICE!

 Completed packets should be turned in to Mr. Deakins @ L2 or Mrs. Myers @ the Gymnasium.  Athletic Directors will be on campus the following dates and times to collect packets prior to the start of school.  All other packets may be cleared when school begins:

8/11 – 8:30-10am (senior orientation), 10:30-12pm (Junior Orientation) and 1-2:30pm (Sophomore Orientation)

8/12 – 9-11am and 4-6:30 pm (Freshman Orientation
